Who are we?

The NHS is currently looking at the best way to provide surgery for children with congenital heart disease (CHD) and has launched a consultation – Safe and Sustainable. As part of this consultation, the NHS is asking to hear from anybody with a view on the future of congenital heart services. As part of this consultation, Ipsos MORI has been asked to undertake the collection of data and undertake the analysis of responses provided on the online (and also paper-based) response forms. What information do we collect about you?

Participation in this consultation is entirely voluntary, and you are free to decide not to take part if you do not want to.

If you do take part, we will collect the information you provide by completing the Online Response Form.

How will we use the information you provide?

The information you provide will form part of the submissions to the consultation and will be analysed for the purposes of understanding participants views regarding the Safe and Sustainable Review of Children’s Congenital Cardiac Services in England.

Please note that the information you provide, including personal information, may be subject to publication or release to other parties or to disclosure under regimes such as the Freedom of Information Act 2000, the Data Protection Act 1998 and the Environmental Information Regulations 2004.

We can assure you that any information you provide during this consultation will NOT be used for marketing, nor will we sell your information to any third party.

How long will we keep the information I provide?

The information you provide will be retained for as long as required for the purposes of this consultation and retained as records as required by law or NHS records of this consultation.

How do you ensure my personal information is secure?

We take very seriously our responsibilities to keep your personal information secure. As such we take every reasonable precaution to ensure your information is protected from loss, theft or misuse. The online consultation web pages use ‘cookies’. These are small files stored on your computer. These files are used as sparingly as possible and only for quality control purposes. We also automatically capture information about your browser type in order to ensure the web-site and response form are delivered in a form suited to the software your computer is using. We do not capture any other information from your computer.

Neither the information collected in cookies nor about your browser type is stored by us. Nor is this information linked by our systems to the personal data or survey responses you provide.
